Why does tracking in separate apps stop working?

What does one all-in-one tracker look like on Home?
The Home screen is the part that decides whether you keep opening the app, so it should answer "what matters today?" quickly. In Planwell, Home shows:
- Today's date, then one nudge at a time. A nudge is about one specific thing, for example a subscription that renews in 3 days, a habit you have not checked in today, a trip that is 6 days away or a show you are still watching. Each has a button you can answer right there, and some offer "Not now" to set them aside. The next nudges are a swipe away.
- An "Ask Planwell anything" bar with Search and Voice buttons beside it, for typing or speaking a question or an entry.
- Two figures, by default your weight and your steps today. Steps come from Apple Health, which is optional and read only.
- Your apps, a grid of cards, one for each tracker you have on. Each shows a key figure, such as a weight or a monthly subscription total, and has a + button that opens that tracker's add screen.
Tap Edit to turn apps on or off and drag them into the order you want. A button under the grid adds more trackers.
Most days you open Home, deal with the nudge, add something with the + button or the ask bar, and close it again.
Which trackers can Planwell hold?
